TEAM is a fun, active program where kids and their families learn about and experience healthier options for eating and being active. There are education sessions as well as opportunity for group activities and one on one follow up. The program consists of the following components:

The Heart Health clinic is a service offered to people who are 18 years of age and older who have risk factors for heart disease or stroke such as high blood pressure, high cholesterol, diabetes, obesity, and /or family history of vascular disease. It is for people who have angina, heart attack, open heart surgery or stroke. Also it is for people who have heart failure or cardiomyopathy.
- Pulmonary Rehabilitation may help you to:
- Reduce your symptoms of shortness of breath and tiredness.
- More easily do daily activities, such as taking a shower and cooking.
- Improve your muscle strength and flexibility.
- Improve your ability to walk and exercise.
The International Travel Clinic offer consultations and vaccines for people traveling outside of Canada. Based on multiple factor including: age, immunization status, pre-existing medical conditions, destination and local conditions it may be advised to receive additional vaccines.
Pregnancy is an important time to gain knowledge and skills for a healthy pregnancy, birth and adjustment to parenthood. Public Health offers a number of programs and supports to help Nova Scotians beginning during pregnancy and thoughout parenthood.

Nova Scotia Health Authority, in a proud community partnership with the Lunenburg County YMCA is pleased to offer this 8 week, comprehensive Cardiac Rehabilitation Program.

BRIDGEWATER & AREA FAMILY HEALTH CLINIC
This is an outpatient clinic directed by Cardiology and lead by a Nurse (RN/NP) with specialty training in Chronic Heart Failure (CHF).
This clinic offers outpatient education for patients and family caregivers living with CHF. Two referral options are available for patients in this clinic: Referrals must be initiated by a physician or nurse practitioner.
